Patient Responsibilities
To ensure high-quality care, patients are expected to actively partner with the EWFHT by fulfilling these responsibilities :
- Mutual Respect: Treat staff with courtesy; a zero-tolerance policy exists for abusive behavior .
- Accurate Information: Provide complete details regarding medical history, medications, and health conditions .
- Active Communication: Engage with providers, ask questions, and report on treatment progress .
- Care Compliance: Adhere to agreed-upon treatment plans and goals .
- Financial Responsibility: Promptly pay any non-OHIP covered fees .
- Facility Use: Use the facility strictly for receiving primary healthcare services .
- Feedback: Express concerns through established procedures .
Note: Failure to meet these responsibilities may result in a review or discharge from EWFHT services .
Patient Rights
As an EWFHT patient, you have the right to :
- Dignity & Respect: Receive compassionate and equitable care in a safe environment .
- Informed Consent: Receive clear information regarding diagnoses, treatments, costs, and risks .
- Right of Refusal: Decline treatment and understand the consequences .
- Privacy: Expect confidentiality, except when legally mandated .
- Record Access: Review or request copies of your personal medical records .
Providing Feedback & Complaints
If you have a concern, please follow this process :
- Step 1 (Resolution): Discuss issues directly with your healthcare team .
- Step 2 (Escalation): Submit written complaints to the Executive Director at 6 Thompson Crescent, Unit 1, Erin, ON N0B 1T0 .
- Step 3 (Timeline): Expect acknowledgment within 5-7 business days and a investigation response within four weeks .
