A Family Health Team (FHT) is a collaborative, patient-centered approach to primary care in Ontario. It connects you with your family doctor alongside a dedicated team of health professionals—such as nurse practitioners, registered nurses, social workers and dietitians – all working together under one roof.
Your care team coordinates your health records and personal care plans, focusing on health promotion, disease prevention, and chronic disease management. FHTs offer several distinct advantages:
