What is a Family Health Team?
A Family Health Team (FHT) is a collaborative, patient-centered approach to primary care in Ontario. It connects you with your family doctor alongside a dedicated team of health professionals—such as nurse practitioners, registered nurses, social workers, dietitians, and pharmacists—all working together under one roof.
Your care team coordinates your health records and personal care plans, focusing on health promotion, disease prevention, and chronic disease management. FHTs offer several distinct advantages:
- Extended Care Hours: Access to team members during evenings and weekends.
- After-Hours Support: Telephone access to registered nurses for non-emergency medical advice when the office is closed.
- Comprehensive Care: Coordinated mental and physical health services in your local community.
