Occupational Therapy services are available at the East Wellington Family Health Team to assist patients of all ages in overcoming physical, cognitive, or emotional barriers. Our goal is to help you solve everyday functional problems so that you can participate fully in your daily activities and live safely and independently.
Areas of Daily Living We Support:
- Self-Care Management: Regaining or preserving independence with dressing, showering, eating, and basic mobility around your home and community.
- Productivity & Daily Activities: Improving your ability to manage meal preparation, household chores, school or workplace tasks, driving, and community transportation.
- Leisure & Recreation: Helping you return to or safely enjoy social interests, gardening, sports, and hobbies.
Specialized Services & Chronic Disease Support
Our Occupational Therapist works both in-clinic and directly within the community to deliver targeted clinical interventions:
- Integrated Care Clinics: Dedicated assessment and treatment support for patients referred to our internal Memory Clinic and Low Back Pain Program.
- Chronic Condition Management: Long-term functional care for individuals living with progressive medical conditions, including dementia, arthritis, COPD, Parkinson’s disease, and Multiple Sclerosis (MS).
- Acute & Post-Operative Care: Targeted recovery strategies following surgeries or acute hand injuries.
- Home Safety & Falls Prevention: Specialized structural evaluations of your living space to eliminate hazards, introduce adaptive equipment, and minimize fall risks.
Caregiver Support:
We work closely with family members and caregivers to develop sustainable strategies for supporting loved ones at home, managing caregiver stress, and ensuring proper body mechanics to prevent injury.

Program Cost & Referrals
No Cost to Patients: This specialized allied health service is fully integrated into your primary care team and is provided at no cost to rostered patients.
How to Access This Program:
Patients can be referred to Occupational Therapy directly by their Family Physician or by another interdisciplinary health care professional working within the Family Health Team. Please discuss a referral at your next scheduled clinic appointment.
