Program Overview
The East Wellington Diabetes Team includes specialized Registered Nurses and Registered Dietitians. We provide comprehensive education and chronic disease management support to help you gain the skills needed to make informed life choices and prevent long-term complications from diabetes.
Program Eligibility Criteria:
- Aged 18 years or older.
- Diagnosed with Prediabetes or any type of Diabetes.
- Requiring insulin starts and adjustments (once official physician insulin orders are received).
Program Limitations Note:
Please note that our team is not insulin pump certified and does not have an ADP (Assistive Devices Program) approved specialist physician on-site.

Workshops and Education Available
We provide both one-on-one appointments and regular group workshops focusing on food choices, physical activity, and medication management. Regular workshop topics include:
- Pre-diabetes: Strategies to prevent your progression to type 2 Diabetes.
- Living Well with Diabetes: Comprehensive lifestyle management.
- Diabetes - Healthy Eating & Label Reading: Navigating nutrition and meal planning.
- Interactive Sessions: Diabetes cooking classes and physical activity advice.
- Clinical Care Skills: Blood sugar testing methods, understanding diabetes medications, and proper foot care.
How to Access This Program
This service is available to both rostered clinic patients and members of the surrounding local community.
