Program Overview
We provide an in-house Substance Use Counsellor dedicated to assisting individuals navigating the complexities of substance use and addiction.
Our services are fully inclusive, offering dedicated help for individuals concerned with their own substance use, as well as family members and loved ones concerned about someone else’s substance use.
Services We Offer
Our program works collaboratively with you to address your unique needs through a variety of support mechanisms:
- Individual Counselling Sessions: Comprehensive assessment, specialized referral, and ongoing solution-focused therapy.
- Goal Identification & Care Planning: Personalized treatment planning incorporating both abstinence and harm reduction approaches depending on your preferences.
- Educational Resources: Specialized information and guidance available to both patients and their support networks.
- Community Partnerships: Joint individual and group counselling services offered in direct partnership with Homewood Community Addiction Services (CADS).
- Expanded Continuum of Care: Direct referrals to specialized CADS support groups, programming in Guelph, and intensive residential or day-treatment programs.
- Regional Connections: Coordination with local community supports outside the Guelph region to ensure your care plan best suits your geographical needs.
How to Access This Program
Eligibility and referral pathways depend on whether or not you are currently rostered with our health team.
Patients of the East Wellington Family Health Team:
You can be referred directly to our in-house Substance Use Counsellor by your family doctor. Please speak with your practitioner or contact our clinic to request an internal referral.

If you are not a registered patient of our Family Health Team, you can still access local addiction services. Please contact HERE 24/7 at 1-844-437-3247 to complete a centralized intake assessment and referral.
