The Rural Wellington Community Team helps connect you to local agencies and navigate complex social programs. We recognize that an active safety net of support is essential for improving physical and mental health, offering a reliable pillar when needed most.
The East Wellington Family Health Team provides a dedicated Outreach Worker, working in tandem with the Canadian Mental Health Association (CMHA-WW) and Home and Community Care Support Services to provide comprehensive support.
